Italy — Healthcare for retirees
Updated
This page is for general information only. It is not medical advice.
What this page covers
How retirees get healthcare in Italy. What the public system is. When you need private insurance. How EU cards and EU pensions work. How paid voluntary enrolment works. What to do if you are from the USA.
Public vs private (simple)
Public healthcare in Italy is the Servizio Sanitario Nazionale (SSN) — the National Health Service. It delivers national essential care levels (LEA) through regional health authorities (ASL / AULSS / ASST).
Private insurance means you pay a company for cover. Many inactive residence permits need private cover or a paid public enrolment.
Coverage is enrolment-based: you need iscrizione al SSN — enrolment in the SSN. That can be:
- Obbligatoria (compulsory / free for listed categories), or
- Volontaria (voluntary / paid annual contribution) — Italy’s clearest “buy into public” product.
Critical map — IDs ≠ free cover:
| Document | What it is | What it alone does not guarantee |
|---|---|---|
| Codice fiscale | Tax ID (required for SSN enrolment) | SSN entitlement |
| Tessera sanitaria | Proof of SSN enrolment; enables GP, prescriptions | Immigration status; cover after permesso expires if not renewed |
| TEAM / EHIC on Italian tessera | Temporary care abroad for obbligatoria enrollees | Residence rights; volontaria enrollees generally do not get Italian TEAM |
AIRE is for Italian citizens registered abroad. Foreign new residents register residenza at the municipal anagrafe and enrol at the ASL — not via AIRE.
Ticket co-pays for specialist visits, diagnostics, and many medicines vary by Region; national exemption frameworks apply (income, age, chronic disease, pregnancy, etc.). Do not invent a single nationwide specialist fee.
If you are from the EU/EEA/Switzerland
Short visits — EHIC / TEAM / GHIC
EHIC means European Health Insurance Card (Italian: TEAM). The UK version is GHIC.
Show EHIC/TEAM (or Provisional Replacement Certificate) plus ID to public / accreditato providers for medically necessary care. Private non-affiliated clinics need not accept EHIC. Dentistry is generally private. Tickets may apply and are not reimbursed in Italy. Emergency medical: 118.
Settling as a pensioner — S1 → ASL
- Obtain portable S1 from the pension-paying competent institution (UK: NHS Overseas Healthcare Services).
- Complete Italian residenza at the anagrafe and obtain a codice fiscale.
- Deliver S1 to the ASL of residence; complete enrolment; choose a medico di medicina generale (MMG) — family doctor.
- Receive tessera sanitaria; care on the same basis as Italian insured persons for LEA in Italy.
No statutory contribution-month waiting period was identified once S1 is registered; administrative delays possible.
Workers and non-active without S1
- Workers / self-employed / registered jobseekers: iscrizione obbligatoria at ASL with employment evidence, CF, ID, residence proof.
- Non-active without S1: stays over three months generally need resources + sickness insurance. EHIC is not a residence document. Residual path: private insurance and/or iscrizione volontaria (same post–2024 minima as non-EU).
If you are from outside the EU
A) Iscrizione obbligatoria (free)
Typical categories: work (subordinate / self-employed) or registered jobseeker; family reasons / reunification; asylum / protection; waiting first work or family permesso; certain pregnancy medical-stay categories; minors; other categories by law/circulars.
Typical documents: valid permesso or renewal/application receipt; codice fiscale; residence/domicile autocertification; ID. Enrol at ASL of residence or domicile on the permesso. Validity tracks the permesso.
B) Iscrizione volontaria (paid — still available in 2026)
Italy’s clearest “buy into public” product for regular stayers >3 months without obbligatoria title — commonly residenza elettiva, religious, volunteering, research, students, au pairs.
Excluded: permesso for tourism or cure mediche — private insurance or full tariffs only.
Annual contribution (L. 213/2023 art. 1 cc. 240–241; ASLs applying 2024–2026), calendar year 1 Jan–31 Dec:
| Category | Annual contribution |
|---|---|
| General (income-based) | 7.50% of previous-year total income up to €20,658.28 + 4% on excess up to €51,645.69; minimum €2,000 |
| Students (no dependents; no income other than Italian public scholarships/subsidies) | €700 flat |
| Au pair / collocati alla pari (no dependents) | €1,200 flat |
| Religious (ASL practice; often €700 flat if no dependents) | €700 |
| If students/au pairs/religious have dependents | General calculation; min €2,000 |
Payment: F24, tax code 8846 + regional code (ask your ASL). Rights start at system enrolment (not payment date); no retroactivity; not fractionable; expires 31 December. Equal treatment for care in Italy while enrolled.
C) STP (irregular)
Urgent/essential care via STP code — distinct from retiree elective-residence pathways.
If you are from the USA
- Medicare usually does not cover routine care in Italy.
- U.S.–Italy SSA totalization coordinates Social Security coverage/benefits — not reciprocal SSN healthcare. No US-issued S1 analogue.
- National visa (e.g. residenza elettiva): consular checklists often require travel medical insurance (example Adelaide Jun 2026 checklist: ≥ €30,000 emergency hospitalisation + repatriation, Schengen-wide) — confirm your competent consulate’s current list.
- After entry: permesso kit / Questura; codice fiscale; anagrafe.
- Non-obbligatoria categories: private sickness/accident/maternity cover or iscrizione volontaria (min €2,000/year for the general case).
- Until enrolled: private insurer is the practical payer; without cover, full tariffs apply.
Simple enrollment checklist
EU / EEA / Swiss (and UK S1-eligible)
- Pathway: worker (obbligatoria) / S1 pensioner / non-active (insurance or volontaria).
- Obtain codice fiscale; register residenza at anagrafe (not AIRE).
- If S1: get form abroad → register at ASL → choose MMG → tessera.
- If worker: bring contract / affiliation evidence to ASL.
- If non-active without S1: private insurance and/or volontaria (F24 8846 + regional code; min €2,000 unless student/au pair rates apply).
- Do not rely on EHIC for settlement; ask ASL about ticket exemptions if eligible.
Non-EU (incl. USA)
- Meet consulate checklist insurance for the national visa.
- Enter; apply for permesso; get CF; anagrafe.
- If obbligatoria category: enrol free at ASL with permesso/receipt + CF + domicile.
- If residenza elettiva / other non-obbligatoria: keep private cover or pay volontaria (min €2,000/year) → ASL → tessera.
- Renew SSN each calendar year (volontaria) or when permesso renews (obbligatoria).
- Tourism/medical-treatment permits: no volontaria.
Things to double-check
- Volontaria minima — post–1 Jan 2024 €2,000 general minimum (not pre-2024 ~€387 figures still circulating on blogs).
- Regional ticket euro amounts — national exemption framework; local price lists — check Region/ASL.
- Consular insurance — Adelaide ≥ €30,000 checklist is one post’s 2026 document; other consulates may demand broader “SSN-equivalent” private policies — confirm locally.
- Corte Cost. 97/2026 — narrow conversion scenario (prior work/family permesso → elective residence after disability pension) per ULSS6; not free SSN for all elective-residence retirees.
- Gazzetta wording of L. 213/2023 cc. 240–241 — consistently cited by multiple ASLs; verify on GU if litigating.
- Volontaria ≠ Italian TEAM — keep travel cover if you need EHIC-style rights abroad.
