Portugal — Healthcare for retirees
Updated
This page is for general information only. It is not medical advice.
What this page covers
How retirees get healthcare in Portugal. What the public system is. When you need private insurance. How EU cards and EU pensions work. Why a user number alone is not enough. What to do if you are from the USA.
Public vs private (simple)
Public healthcare in Portugal is the Serviço Nacional de Saúde (SNS) — the National Health Service. It covers mainland public care. The Azores and Madeira run separate regional services.
Private insurance means you pay a company for cover. Visas and residence often need it until your SNS record is complete.
Anyone who is ill can be assisted at a health centre or hospital. Who pays depends on legal status and whether your SNS record is complete.
Critical distinction — NNU ≠ SNS pays. The número nacional de utente (NNU) is the national health user number. It is created when you are entered in the Registo Nacional de Utentes (RNU) — the national user register. Having an NNU alone does not mean SNS covers the bill. For SNS cost coverage, the record generally needs an updated (registo atualizado) set of: identification, Portuguese NIF (tax number), full Portuguese address, and a valid residence authorisation (AR) (lighter rules for minors).
RNU status (ERS / Despacho rules):
- Registo atualizado (updated) — SNS generally assumes financial responsibility; needed to enrol in primary care.
- Registo em curso (in progress) — incomplete for up to 180 days after SNS contact while you finish updating data.
- Registo incompleto (incomplete) — after that window if data are still missing: SNS no longer assumes usual financial responsibility; primary-care enrolment is blocked.
There is no verified Spain-style paid public buy-in (convenio especial) for general retirees in Portugal. Public access for legal residents hinges on updated RNU, not a flat fee schedule.
If you are from the EU/EEA/Switzerland
Short visits — EHIC / GHIC
EHIC means European Health Insurance Card. The UK version is GHIC.
Show a valid card for medically necessary, unplanned care in public SNS units (and some SNS-conventioned providers). If you lack the card, request a Provisional Replacement Certificate from your home institution. Planned care needs S2. Most SNS acts no longer carry taxas moderadoras (user fees); non-referred hospital emergency may still charge a fee. Medicines often paid upfront with reclaim at home. Emergency: 112; clinical advice: SNS 24 — 808 24 24 24.
Settling as a pensioner — S1 → Segurança Social → centro de saúde
- Obtain portable S1 from the pension-paying competent institution (UK State Pension: NHS Overseas Healthcare Services).
- Submit S1 to the Centro Distrital of Instituto da Segurança Social for your area.
- ISS validates S1, returns a copy, and points you to the local centro de saúde (health centre).
- Present the validated S1 at the centro de saúde; complete RNU registration and receive your NNU.
- Enrol in primary care once registration is updated; family-doctor assignment depends on local vacancies.
People who receive both a foreign Member State pension and a Portuguese pension while resident in Portugal are treated as SNS users via Portuguese social security without a portable certificate. Workers insured under Portuguese Segurança Social (NISS — social-security identification number) enrol as ordinary SNS users.
Non-active EU residents without S1: with a municipal certificado de registo de residência, register at the local centro de saúde under residence rules. Stays over three months without economic activity generally also need sufficient resources and sickness insurance where free-movement rules require it — confirm current Câmara / AIMA practice. EHIC is not a residence document.
No statutory contribution-month waiting period was identified once the correct entitlement document is registered; appointment and vacancy delays are operational.
If you are from outside the EU
- Enter / stay legally (national visa → autorização de residência via AIMA, or other lawful basis).
- Obtain NIF (and NISS if employment or AIMA asks).
- Register in RNU at first SNS contact (or via combined Espaço Cidadão service where eligible).
- Ensure registo atualizado: ID + NIF + full address + valid AR.
- Enrol in primary care at the local centro de saúde / USF; request a family doctor when vacancies exist.
Manual note: NISS is not required merely to obtain an SNS user number. Holders of AR application/renewal receipts may continue SNS access on equal terms while the receipt is valid — confirm current AIMA practice.
AIMA Art. 77: residence authorisation generally requires health insurance or proof of SNS coverage. AIMA FAQ: adequate disease protection can be shown by an authenticated SNS enrolment declaration or an insurer declaration (national or authenticated foreign).
No open paid public buy-in for general third-country retirees was verified. Without valid AR, expect self-pay except urgent/vital and listed public-health / maternal-child / minors / vaccination / social-exclusion categories.
If you are from the USA
- Medicare usually does not cover routine care in Portugal.
- There is no USA–Portugal bilateral SNS healthcare agreement. U.S.–Portugal SSA totalization coordinates retirement / survivors / disability benefits only — not reciprocal SNS health insurance.
- Visa stage: expect travel/medical insurance covering necessary care, urgent assistance, and often repatriation — confirm the current consulate / vistos.mne.gov.pt checklist (exact minima vary by post; do not assume Schengen €30,000 automatically binds every national-visa file).
- AIMA stage: insurance or SNS proof (Art. 77).
- After lawful residence: register for NNU / updated RNU with AR + NIF + address; enrol at the centro de saúde. Public SNS then follows the same updated-registration framework as other legal third-country residents — not via a US portable form like S1.
- Until updated registration is in place: private insurance / self-pay. EHIC/GHIC does not apply to US-insured persons.
Simple enrollment checklist
EU / EEA / Swiss (and UK S1-eligible)
- Pathway: worker (Portuguese social security) / S1 pensioner / non-active (municipal certificate).
- Obtain NIF; complete residence registration (Câmara EU certificate or residence card).
- If S1: request abroad → Centro Distrital Segurança Social → keep validated copy → centro de saúde.
- Confirm RNU registo atualizado and receive número de utente.
- Request primary-care inscrição / family doctor (vacancy-dependent).
- Optional: SNS 24 portal; Portuguese CESD/EHIC via Segurança Social Direta when eligible.
- Optional private top-up (dental, repatriation, private networks).
Non-EU (incl. USA)
- Meet consulate insurance wording for the national visa.
- Enter; book AIMA; prepare Art. 77 file (insurance or SNS proof).
- Obtain NIF; obtain NISS if applicable.
- Optional combined Espaço Cidadão request (NIF + NISS + SNS number) if eligible.
- Attend public unit; complete updated RNU with valid AR.
- Enrol in primary care; keep private cover until SNS proof is solid.
- Watch the 180-day incomplete-registration window.
Things to double-check
- NNU alone ≠ SNS pays — incomplete RNU after 180 days shifts cost liability and blocks primary-care enrolment.
- No verified paid public buy-in for general third-country retirees (unlike Spain convenio / Italy volontaria).
- MNE national-visa insurance minima — confirm at the competent consulate; research could not lock a single universal euro floor from MNE pages.
- Family-doctor waits are capacity constraints, not statutory insurance waiting periods.
- Azores / Madeira — regional health services; do not assume identical mainland desk procedures.
- Older GOV.UK fee examples (€5 GP / €15 A&E) predate abolition of most taxas moderadoras (DL 37/2022) — prefer ERS/SNS.
